Subscription Services
Your Valeon Health membership begins once you complete your intake form and your first monthly payment is processed.
Your subscription may include access to licensed medical providers, clinical review, care support, lab services where applicable, prescription medication when clinically appropriate, and other services designed to support your health and weight loss journey.
Prescription medications are only provided when deemed appropriate by a licensed medical provider. Completing an intake form does not guarantee eligibility, approval, treatment, medication, or specific results.
If a Valeon medical provider determines that you are not eligible for treatment for medical reasons, you will receive a refund in accordance with the policy below.
Valeon does not guarantee any specific outcome, result, amount of weight loss, health improvement, or medication effectiveness. Results vary by individual and depend on many factors, including medical history, adherence, lifestyle, and provider recommendations.
Cancellation Policy
You may cancel your Valeon Health subscription at any time.
To cancel, please contact Valeon support at support@valeonhealth.com or submit a cancellation request through your patient portal, if available.
To avoid being charged for your next billing cycle, cancellation requests must be received at least 72 hours before your next billing date.
If your cancellation request is received less than 72 hours before your next billing date, your subscription may still renew for the upcoming billing cycle, and your cancellation will take effect at the end of that next billing cycle.
After cancellation, you will continue to have access to your Subscription Services through the end of your current paid billing period, unless otherwise required by law or determined by Valeon for safety, medical, or compliance reasons.

Refund Policy
Medical Disqualification
If a Valeon medical provider determines that you are not eligible for treatment for medical reasons during your initial medical review, you will receive a full refund of your Subscription Services payment.
If you are later determined to no longer be eligible for the Subscription Services after treatment has begun, your subscription may be canceled. Any refund, if applicable, will be limited to the unused portion of your most recent billing cycle and will be determined based on services already rendered, medication orders already placed, and applicable law.
General Cancellation Refunds
Except in cases of medical disqualification or as otherwise required by law, Valeon does not issue refunds upon cancellation of Subscription Services.
This includes, but is not limited to, cancellations due to change of mind, lack of use, missed appointments, delays caused by incomplete patient information, failure to complete required labs or forms, or not achieving a desired result.
Subscription fees cover access to clinical review, provider services, care support, administrative services, and, where appropriate, medication coordination. Once these services have been initiated, fees are generally non-refundable.
Prescription Medication Refunds
Federal and state laws generally prohibit prescription medications from being returned to a pharmacy for refund, resale, or reuse once they have been dispensed or shipped to a patient. This rule exists to protect patient safety and public health.
Because of this, Valeon cannot guarantee refunds for prescription medications once they have been ordered, dispensed, shipped, or delivered.
If a refund is approved, it will apply only to your most recent billing cycle. Refunds will not be issued for prior billing cycles.
Damaged or Incorrect Medication
Please inspect your medication immediately upon receipt.
If your medication appears damaged, defective, incorrect, or otherwise compromised, contact Valeon support immediately at support@valeonhealth.com
Damaged Items
Damaged medications may be replaced by the dispensing pharmacy after review of the issue and receipt of sufficient evidence of damage, such as photographs, packaging details, or other requested documentation.
Incorrect Items
Incorrect medications will be reviewed and, where appropriate, replaced by the dispensing pharmacy.
Do not use any medication that appears damaged, incorrect, tampered with, or different from what your provider prescribed. Contact Valeon support before taking the medication.

What is included in my Subscription Services price?
Your Valeon Health subscription represents a single monthly price for the Subscription Services you purchased.
Depending on your plan and eligibility, this may include provider consultations, asynchronous medical review, prescription medication when clinically appropriate, pharmacy coordination, lab services where applicable, care support, and other services related to your medical weight loss program.
The specific services, labs, medications, and support included may vary by plan, location, medical eligibility, pharmacy availability, and provider determination.
What if my provider determines I am no longer eligible?
If your Valeon provider determines that you are no longer eligible for the Subscription Services, your subscription may be canceled for medical or safety reasons.
If your cancellation is initiated due to provider-determined medical ineligibility, a refund may be issued in accordance with this policy.

In all cases, you remain responsible for fees associated with services, healthcare services, provider review, pharmacy coordination, lab services, and other services already rendered. This Cancellation and Refund Policy applies only to fees for Valeon Health Subscription Services.
